Helpline complaints form 

Whilst we always strive to provide an excellent service, we know that from time to time mistakes can happen. We want to be able to resolve them and learn from them to prevent them happening again.

If you have an issue related to the service delivered via the Helpline, please raise it by providing the following details  

If you are reporting a Helpline complaint on behalf of someone else (e.g. a colleague, employee or family member) please note that due to the confidential nature of the Helpline service and in order to protect privacy we will require the individual’s consent (via a signed form) to keep you updated.

Without a signed consent form we will still investigate and the team will reach out directly to the individual to follow up (if appropriate and possible). Please understand however, that we cannot keep you updated regarding the investigation without the signed consent form.
